Take A Survey!

A friend of mine is Graduate student at Tufts and she needs as many responses as she can get. So please take the survey below - it's quick, easy to follow, and will help her a lot! - Petitedov

For a human factors class I'm taking this term at Tufts, my group is charged with the task of redesigning the electronic medical records system in the U.S. It's quite an overwhelming task, and so I'm trying to draw upon as many resources and people as I can who could provide their input and perspective.

My group is currently circulating two different questionnaires to help guide our design: #1 is for patients and #2 is for "health care providers".

1) The first questionnaire is intended to be filled out by patients. Luckily everyone qualifies as a patient. So if you can spare 5-10 minutes of your time and fill out our online survey, I would be very appreciative! (However, if you consider yourself a health care provider (see below) please fill out questionnaire #2 instead).

2) The second questionnaire is aimed at healthcare providers. This group includes: physicians, nurses, pharmacists, doctor/hospital administrative staff, counselors, medical students (assuming they have some rotations experience), EMTs, insurance providers, etc. If you fall into this category, I would be very appreciative if you could fill out this questionnaire instead of the patient one.
